MTBK Massage Membership

This Monthly Membership Agreement is entered into between Massage Therapy by Krissy and the undersigned individual. By enrolling in a membership, the member agrees to the following terms and conditions:

1. Membership Benefits:

• Members may choose from the following membership options:

1. 60-minute membership: One 60-minute session per month (massage or Level 1 Signature Facial)
2. 90-minute membership: One (1) 90-minute massage per month.
3. 120-minute membership: One (1) 120-minute massage per month.

• Bring a friend to receive your membership rate on their first visit
• Choose a massage or Level 1 Signature Facial each month (60-minute plan only)
• Membership credits roll over for up to 4 months
• 10% off timed add-on services and specialty massage therapies
• 10% off retail products
• 10% off other facial services
• Members have 60 days to use any credits from a cancelled or frozen membership.
• Credits may be gifted to a friend or family member
• Unused credits may be applied toward esthetician services, add-ons, or specialty offerings

2. Membership Fee & Payment:

• 60-minute membership: $85 per month
• 90-minute membership: $132 per month
• 120-minute membership: $180 per month
• Payment is automatically charged to the member’s designated payment method on the same date each month
• A declined payment must be resolved within 3 days or membership benefits may be suspended

3. Appointment Scheduling & Cancellation Policy:

• Members are responsible for scheduling their monthly massage.
• A minimum 24-hour notice is required for cancellations or rescheduling.
• Late cancellations or no-shows will result in the forfeiture of that month’s session

4. Membership Term & Cancellation:

• This is a month-to-month membership with a minimum three-month commitment
• If a member cancels before the three-month term, they will be charged the standard non-member rate for previous sessions
• Membership auto-renews unless a written cancellation is received at least 15 days before the next billing date
• Cancellations must be submitted in writing; phone cancellations are not accepted

5. Freezing or Pausing Membership:

• Members may request to freeze their membership for up to two (3) months per calendar year.
• Freezing a membership must be requested in writing, and no charges will occur during the freeze period.
• Unused massages will remain available for use 60 days after the membership resumes.

6. Liability Waiver:

• Members understand that massage therapy is not a substitute for medical care.
• Members agree to disclose any relevant health conditions prior to treatment.
• Massage Therapy by Krissy is not responsible for any issues arising from undisclosed conditions.

7. Modifications & Termination:

• Provider reserves the right to modify membership terms with 30 days’ notice.
• Provider may terminate this Agreement at any time if the Member violates policies or engages in inappropriate behavior.

8. Agreement Acknowledgment:

 By signing our Member agreement, the Member acknowledges that they have read, understand, and agree to the terms outlined in this Agreement.
